Purpose
The purpose of SICIL's Community Reintegration Program is to provide services to help people with disabilities that are living in nursing homes and are under the age of 60 years of age return to community living.
Service Coordination
CICIL staff assist eligible consumers in finding and securing the community residence. Possible one-time expenses may include initial rent and/utility deposits, furnishings. Small appliances, stock of food, etc. Purchases of equipment, assistive devices, and home modification for accessibility may be part of the project.
Training & Support
Staff will work with each consumer as he/she develops the skills and confidence to manage and maintain his or her independence in the community. Training is also available to PA's, family members, and significant others who may help smooth the transition from institutional to community living.
